On mine the reticle 'fell down'. Obvious ones: when the lenses come apart, fall out of their mount or it gets a leak and condensation gets inside. A less obvious fail is when the adjustments don't work right anymore or an optic becomes loose in the scope. On those it is only really obvious if the scope does not hold zero or an adjustment fails to make the proper change of POI.
